When Death Occurs
- Call Salm-McGill and Tangeman Funeral Home (937) 492-5130
- Provide us with the following information
- Your Phone Number
- Who has passed and what time
- The location of the deceased
- What the physician’s name is
- Make an Appointment with us at Salm-McGill and Tangeman Funeral Home
- Notify your clergy
- Gather information for the Death Certificate:
- Full name of deceased
- Deceased address
- Occupation of the deceased
- Date and place of birth
- Social Insurance/Medicare number
- Marital Status
- Father’s name and birth place
- Mother’s name and mother’s maiden name and birthplace
- Several recent photos
- Clothing or personal items for the deceased
- Full set of clothing and undergarments
- Eyeglasses, dentures, jewelry, etc
- Religious symbols ( cross, rosary, etc)
- Gather Obituary Information: Salm-McGill Tangeman Funeral Home will write the obituary for you to submit to the newspapers
- Decide an appropriate memorial to which gifts can be made. ( Church, Hospice, School, or Charity)
- Locate the will and notify the Lawyer and Executor
- Check and notify:
- All Life Insurance Companies
- Death benefits
- Investment Company
- Bank, Trust Company or Credit Union
- Trade Union
- Fraternal Organization
- Military Organization (Legion, Veterans of Foreign Wars)
- If deceased was living alone, notify:
- Landlord
- Utility Companies
- Post Office – forwarding address
- Select Pallbearers
- Notify any distant person you want to be notified
